Application Form

DEADLINES ~ Prospective students may submit applications at any time, at least one month before the beginning of the next module. Prospective students can register for short courses even before the completion of the registration and will be credited for attending them.

Please right-click here to download a copy of our application form. Please read this application form carefully. Be sure to include all of the required enclosures, the completed checklist, and the application fees (you can pay through the PayPal "Add to Cart" button below).

Applications may be emailed or sent on a CD/DVD. Artwork images should be burned to a CD/DVD.
